I understand that Journeymen Triangle programs are designed to support the social, emotional, and personal development of participants through mentoring, group discussions, outdoor experiences, leadership activities, service projects, physical activities, team-building exercises, and other educational or experiential programming.
I understand that participation in Journeymen Triangle programs may involve physical, emotional, intellectual, and social activities that carry inherent risks. These risks may include, but are not limited to:
I understand that no activity can be entirely free from risk and that unforeseen circumstances may arise during participation.
I voluntarily choose to participate, or permit my child to participate, in Journeymen Triangle programming with full knowledge of these risks.
Participation in Journeymen Triangle programs is voluntary. Participants are encouraged to engage at their own pace and may decline to participate in any activity that causes discomfort or exceeds their personal limits, except where participation is required for safety reasons.
I authorize Journeymen Triangle staff, volunteers, and representatives to obtain emergency medical treatment for myself or my child if reasonable efforts to contact a parent, guardian, or emergency contact are unsuccessful or if immediate medical attention is necessary.
I understand that I am responsible for any costs associated with medical treatment, transportation, or emergency services.
I certify that all medical, behavioral, emotional, and health information provided to Journeymen Triangle is accurate and complete to the best of my knowledge.
To the fullest extent permitted by law, I release, waive, and discharge Journeymen Triangle, its officers, directors, employees, contractors, volunteers, mentors, facilitators, partner organizations, and agents from any claims, liabilities, damages, losses, or expenses arising from participation in Journeymen Triangle programs, except in cases of gross negligence or willful misconduct.
This release applies to all Journeymen Triangle activities, including but not limited to group meetings, mentoring sessions, school-based programs, outdoor activities, retreats, workshops, service projects, transportation associated with programming, and special events.
Journeymen Triangle is not responsible for lost, stolen, or damaged personal belongings brought to programs or events.
Journeymen Triangle encourages participants to share openly and respectfully within program settings. Participants are expected to honor the privacy of others and maintain confidentiality regarding personal information shared during group activities.
However, confidentiality cannot be guaranteed in circumstances involving:
Journeymen Triangle staff and volunteers are required to comply with all applicable laws regarding mandatory reporting and participant safety.
I grant permission for Journeymen Triangle to use photographs, video recordings, or other media that may include myself or my child for educational, promotional, or organizational purposes.
